Synopsis: This poster demonstrates one community's use of Concept Mapping results to form a Logic Model for a Phase V System of Care in Hattiesburg, Mississippi, commUNITY cares. Concept Mapping was used as a community engagement tool to provide direct community input into the Logic Model creation process. The result is an engaged community with a Logic Model that serves as a compass for commUNITY cares.
