Synopsis: Implementation of the system of care in Montgomery County, Maryland has been lead by the Collaboration Council and partnerships with the Department of Health and Human Services, the state of Maryland, the mental health system and others. Maryland Choices serves as the Care Management Entity responsible for care coordination services and management of the provider network. Additional components of Montgomery County’s implementation, including a family support organization and youth involvement, will be discussed. nally, DHHS supported the formation of the county’s family support organization. The Collaboration Council has successfully identified additional funding sources that allow families, school personnel, and other community members to refer youth to wraparound before they are at risk for the most restrictive levels of care. Another critical component of the successful implementation and sustainability of Montgomery County’s system of care is a Local Access Mechanism that screens youth and makes referrals to wraparound via a Local Coordinating Council. The effectiveness of system of care implementation in Montgomery County is being assessed using the Wraparound Fidelity Index 4.0 (WFI) and the Child and Adolescent Needs and Strengths (CANS) assessment. Results from the WFI indicate that Choices has good fidelity to the principles of wraparound. CANS scores indicate that youth and families experience improved functioning while in wraparound as exhibited by the development of strengths and a reduction in needs across life domains.
