Synopsis: Asian Americans traditionally have the lowest utilization of mental health services when compared to all other racial and ethnic groups. The Bridge Program is an innovative program that bridges the gap between primary care and mental health services to improve access, improve workforce capacity, and raise community awareness for this population. This presentation will provide an overview of the program, discuss essential determinants for its success, and share ideas for replication.

Synopsis: Individuals and families from diverse communities more readily seek help for physical and mental health problems in primary care settings. This session will present different models of integrating mental health and primary care, challenges and solutions, and strategies for documenting improved access, quality and outcomes of care.
