Synopsis: Communities must go through a transformation process in order to effectively implement systems of care. National, state and local funders of these systems, as well as organizations charged with supporting them through training, coaching and mentoring, need to have information on where communities are in this transformation process in order to maximize their resources. This presentation describes an assessment and rating process that uses change theories to identify the level of system of care development. Results based on three years of data from over 35 communities in Indiana will be presented and discussed.
