Synopsis: To ensure effective services for families with children in multiple systems, families and communities must come together to be able to create care systems. This discussion provides the outcome data from two communities who have been successful in bridging services for multi-problem families. Systems involved are mental health, juvenile justice, child welfare, schools, and other community organizations. Community mental health centers in both communities took lead roles. One community developed creative wraparound services and the other developed and intense home-based treatment model. There will be opportunities for participants to engage in an interactive discussion and an experiential activity for developing systems in simulated communities.
