Synopsis: Starting Early Starting Smart demonstrations integrated child and caregiver behavioral health services into service homes. This study examines the sustainability of 8 of these programs through telephone interviews with key informants and coding final reports. Three of the programs fully sustained services and principles; four partially sustained with reductions and/or modifications; one program institutionalized service principles only. The study emphasizes the importance of program quality and procedures, such as staff roles, skills and continuity, for sustaining services and principles. When programs sustain funding through replacement sources, funding requirements to modify services often conflict with sustaining service and operational innovations.
