Synopsis: This paper discusses the findings from an analysis of three cohorts of grant communities which completed their grant cycles in 2003 through 2005. This study sought to determine how well each of these “graduating communities” implemented a system of care by examining existing data. The study further sought to identify factors that either facilitated or inhibited the implementation of a system of care. The problems facing the system of care grantees proved to be similar to those of earlier demonstration programs such as Model Cities. A framework for understanding these common problems is presented along with recommendations for policy makers.
