Synopsis: We examined age of onset; strength of objective, subjective and global caregiver strain; severity of functional impairment; and severity of emotional distress to determine if those adolescents that continue substance use after one year of System of Care (SOC) participation differ from those who do not continue to use. The MANOVA was significant (F (9, 45) = 2.63, p<0.05) and groups differed in terms of objective (p<0.045), internal (p<0.003), and global caregiver strain (p<0.041). Findings support the conclusion that ongoing substance use is associated with elevated caregiver strain. While we cannot posit causality, caregiver strain should be considered an important issue in future SOC programs.
